Eyelid Lifts are probably one of the most high impact procedures that you can get. "The eyes have one language everywhere" according to 16th century English poet George Herbert and we all know how important they are in communicating our feelings. Our clients have found that eyelifts have dramatically changed how they feel about themselves and how others perceive them. The most frequent feedback from our patients is their amazement that no one can tell that the patient had surgery. "Did you change your hairstyle? Are you back from a vacation? Is that new makeup you´re wearing?" These are the questions that most people will ask you after an eyelift.

Upper lid blepharoplasties are performed to get rid of the puffiness (puffy eyes) or hooding of the eyes. Lower lid blepharoplasties address the bags (buggy eyes)or festoons that appear under the eyes that give a person an angry or exhausted expression. Upper lid blepharoplasties are performed through the lid crease and the scar is well hidden whether the eyes are open or closed. Women will find that eyeshadow is much more easily applied. All patients will find that their face is significantly rejuvenated and they will be described as "energetic". Lower lid blepharoplasties can be performed in two methods. Depending on skin type and ethnic background, if the skin is papery thin, the procedure may be done along the lower lash line and the scar is well hidden there. Most patients however, qualify for a "transconjunctival" approach which means that the procedure is done on the inside of the lower lid so no visible scar will appear.

Blepharoplasties are one of my most highly sought after procedures, owing to the fact that I am a superspecialist in this area. I have training first in eye microsurgery (ophthalmology) and then further training in Oculofacial Plastic and Reconstructive Surgery as well as in Facial Plastic Surgery. I have performed numerous revision surgeries for patients who were dissatisfied by procedures that they had performed elsewhere.

This 60 year old man wanted to get his upper eyelids done so that he could get the culturally prized upper lid crease. He felt that the upper lid surgery would make his eyes look larger. He was not interested in lower lid surgery.

Ethnic eyelids are also a specialty of mine. Asian, African, and all other types of eyelids have unique qualities that are attractive and must be preserved. I have written articles in this area in peer reviewed journals and I have performed hundreds of double lid procedures for Asians who would like to have an upper lid crease but are intent on preserving the beautiful attributes of the Asian eye.

This 43 year old male wanted to look younger but did not want a drastic change. He chose to get lower lid blepharoplasties to get rid of the swelling under his eyes which he felt aged him quickly.

Other important subtleties to understand are the differences between the male and female eye and how it relates to the eyebrow. In females, there is an arch that is approximately central to the eyebrow. The male eyebrow tends to have a slight rise in the lateral third. These gender differences must be preserved or else you will see patients with cookie-cutter´ eyes that are too round and look perpetually surprised. If you look at celebrity magazines with this awareness, you will be able to spot actors who received feminized or doll´s round eyes from poorly performed blepharoplasties.

This 55 year old stockbroker felt that she needed a lift to make herself feel more youthful. The upper lids have a clear, discernible crease, makeup is applied more easily and the eyes appear “larger”. The lower lids have the pouches removed. The fine lines will not be improved by surgery but other modalities such as intense pulsed light would make a big difference.

Once you have done your "homework" and studied the types of blepharoplasties that are out there, you should have a clear idea of what you want. I am adamant that my procedures have a natural look that makes you look younger but not over pulled. A 60 year old man has a good chance of looking like a refreshed and energetic 50 something year old man but any younger would be quite unrealistic.
